Summary Cause Forms
The following forms are to be utilised for preparing a Summary Cause Action to the Sheriff Courts in Scotland. All the forms are available in PDF format (which requires Adobe Acrobat software to view on your computer). Forms 1a - 1d are avilable in MS Word format, and the rest of the forms in Rich Text Format (RTF).
The forms may either be completed and printed by filling in the relevant details in the boxes highlighted in blue , or simply printed so that the details can be hand written into the relevant boxes.
Note: If you wish to save changes you make to these forms on your local system you will require to purchase and install the full version of the Acrobat software, available from Adobe's website.
Later this year we will make available the option to submit these applications via e-mail. This will require the pursuer (or his representative) to hold an account with the Sheriff Court. Initially it will be a requirement that all applications be printed and submitted to the courts on paper.
Law firms and individuals who wish to download a copy of the forms and store these on their own computers may do so but no attempt should be made to alter the layout of the forms in any way without prior approval. For further advice on the reproduction of these forms please contact:
